By completing and submitting this form you are agreeing to the terms of the SurveyMonkey CCPA Addendum on behalf of the Customer and you are confirming that you (or the organisation/person on whose behalf you are completing this form) have a Qualifying Account to receive this CCPA Addendum. You are also confirming you have the legal authority or capacity to accept the CCPA Addendum on behalf of the Customer. The primary admin/account holder will receive an email confirming that this form has been completed. The Account Holder is responsible for obtaining legal advice regarding whether the CCPA Addendum satisfies the legal obligations of the relevant SurveyMonkey Account Holder and SurveyMonkey makes no representations and provides no guarantee that this CCPA Addendum will satisfy the legal obligations of the person or organization downloading the CCPA Addendum.
